Forgot phone's lock code in motorola W755

My friend has a Motorola W755 mobile phone and from two days the phone is locked, he is not able to make a call or do some thing with it only he receives call from other users. please provide some solution to unlock the motorola phone any suggestion to unlock the phone will be appreciated.

I have tried all the combination's that you have provided above but but the problem is the same. The phone is locked still now and i am suffering from it badly, now the calls from the others user are not coming on the phone.So, please provide a proper solution to overcome these issue any suggestion on the topic will be helpful.
