YOu can use this software it will give all the details of your machine. Do let me know if you have any issues.

Otherwise, go to belarc.com and download the free advisor. It does a pretty good job of identifying motherboard models.

It must have been mentioned on the board itself, normally near the middle or the expansion slots.
Else, CPU-Z / Belarc Advisor will be able to tell you some information about the board too. I have used Belarc Advisor and its a pretty good tool that gives detail information about PC hardware and software as well.
