sample.exe stopping shutdown

I have recently reformatted my computer. There is no spyware or any viruses present. When ever I just shut down my computer, then I am getting 2 successive windows saying Sample.exe end process and I have to click end process. What is this thing, does anyone know about it? Thanks for any help.
