After trying for hours with no luck despite trying every solution posted here and all over the internet I was finally able to discover the solution.
Uninstall KB898461 from the add/remove programs.
Not sure when it got there or why it wasn't doing its job, but removing it seemed to fix everything.

Hi, I can't install that patch either, for weeks now. I've tried everything in this thread, but it still won't help.
When I try to download the patch file and install it directly, it claims to have been installed successfully, but the patch is the only one that keeps showing up in the auto updater. There it fails and produces this log:

KB898461 consistently fails to install, updates wont work simply update Windows Installer 3.1 to Windows Installer 4.5 it is available here

I only joined to post what finally worked for me.
Got rid of viruses/malware.
Tried several other suggestions that didn't work.
1. Ran QFECHECK.EXE to verify Windows patches/updates had installed correctly.

2. Deleted the C:\WINDOWS\$hf_mig$\KB898461 folder.

3. Downloaded and installed the KB898461 update successfully!
4. Went to Windows Update and it's downloading updates now.
I also stopped Norton AV, but not sure if that matters.
