The form can be of any type. For example like registering in forum like in this one, its a kind of form in HTML, or it can be for Contacts as you said, or any such form; and the codes required for such tasks. I have got some valuable information from the link that JamesMK gave. Still, if you can add something to my knowledge, that will be appreciable :)
