You had not explained properly what actual template you want. What I can figure out that there are number of templates for every purpose. There is no composite option available. Anyhow I found the template linked provided below. Download it and check it out. Also you can just edit or modify the template as per your need.

I can assume that you can try the below template. It is a billing template. For the one which you are asking is much easier to maintain in Microsoft Excel. You can simply create columns and add info as and when needed.
