Tool to display perfmon captured data

I have my perfmon data going to a SQL database. Now I would like to throw a
front end Web based interface to it so we can look at historical performance
by choosing the server and respective counters.

I dont know how to code and hence want to know if there are any tools out
there that I could download to connect to this database and chart my data ?

There are many reporting tools you can use against SQL Server. SQL Server
comes with reporting services so you might want to give that a go.
