According to me everything seems to be OK. Starting with SNR, it is the level of a desired signal to the level of background noise. The higher the ratio, the less obtrusive the background noise is. So its proper for cable modem. Then talking about frequency, that is also not bad. So I don't think there any problem with your modem, check if your settings are proper.

What operating system are you having? If you are having Windows Vista then either it is not supported or you are missing the correct drivers for your modem. Also can you see the modem in the Device Manager? If yes then check if there is any kind of yellow exclamation mark asking for drivers.

This could be the problem of a software installed recently corrupted or interfering with your modem drivers. Uninstall any such software and also do a virus scan if that comes out to be some malware activity. If everything comes out to be positive then I suppose your modem is dieing and you need a replacement.
