Apparently your video card is overheating, the fan see if it works, could also be the source of power is not powerful enough.

Your Test results are reporting that the temperatures are normal, another possibility is that your power is not the necessary capacity and not enough energy to reach this card.

If I remember this card needs at least 500 watts with 25 amperes per actual output 12V.It would be good to revise it.
