Service Level Agreement with an ISP i.e. Internet Service provider

I am looking for a sample Service Level Agreement that I can modify and present to out Internet Service Provider. This has been necessitated by unbearable down times due to what the servide prodiver is calling "Base Station" problems. We could easily walk away were it not for the fact that we had invested in Wimax radio equipment which is not compatible with other providers hardware.

What is your location? and which ISP you are using? If you'll go for small local ISP providers then its common to face problems like down times even with leased line connection.
