ACCOUNT IS LOCKED REPEATEDLY

Hello MVP guys, I have an AD over Win 2003 R2 SP2, some accounts are locked
repeatedly and someones whit password whit mark never expire, what is the
possible problem whit it.

Some one had any problem like this?
